A map indicating the proposed location of the plant office and the point where supply is required.
The map should normally be of the scale of 1 cm representing 1200 cm.
Licence / NOC from statutory authority, if required, or a
declaration that the connection does not fall under NOC requirement under any statute.
For proprietary firm: Affidavit stating that the applicant is the sole proprietor.
For partnership firm: Partnership deed.
For Limited Company: Memorandum & Articles of Association and Certificate of Incorporation.
Proof of permanent residential address of the consumer and PAN (if any).
Any subsequent change must be intimated to the Licensee immediately.
Letter of intent for production / enhancement in production (if available).
List of equipment proposed to be installed along with the expected load.
Resolution for authorized signatory.
Registration from Industries Department.
Additional Information Required
Provide tentative date(s) from which each phase of Contract Demand is required.
